The end-of-life is never easy and every moment matters. The memory of your loved one’s passing will remain with you forever. Let us hold your hand through this difficult time and alleviate some of the stress associated with this journey.

How We Work With Hospice

Your hospice care team will consist of a social worker, nurse, aide and chaplain who visit to provide your loved one with medical, emotional and spiritual support. They are available to you by phone 24 hours per day, 7 days per week.

The hospice nurse will develop a care plan indicating the services to be provided and frequency of visits. The nurse will visit at least once every two weeks; the social worker and chaplain will visit once or twice per month; the hospice aide will visit two-to-three times per week for 30 minutes to an hour; care during all other times is typically provided by the family.

For some family members, caring for their loved one at the end of their life can bring a sense of purpose and fulfillment. For others, this round-the-clock care can be difficult and quickly become exhausting, possibly leading to caregiver burnout

How We Can Help Your Family

Preferred Care at Home of Central New Jersey fills the gaps when hospice can’t be there. Our caregivers are specially trained in the practical and emotional aspects of end-of-life care and will create a peaceful and comfortable setting during the final days.

Our goal is to be present without intruding, provide calm support to the entire family, and maintain your loved one’s dignity during this difficult time.

Below is a list of some of the services we provide:

  • Communicating and tracking medical changes
  • Perform bed baths and moisturize skin
  • Help with toileting or incontinence
  • Monitor skin integrity, reposition to prevent skin injuries, care for pressure wounds
  • Assist with medications
  • Prepare meals and help with eating
  • Wash laundry and perform light housekeeping, including change the bed linens while your loved one remains in the bed
  • Play soothing music, read, or have a conversation
  • Sometimes the most important thing is simply sitting with someone and holding their hand.

Our flexible scheduling provides peace of mind that we’ll be there if you need to rest or step away. You choose the number of days and hours we spend with your loved one. For some clients we spend as little as 15 hours per week. For others, more care may be required.

Some families request overnight or live-in care so they have the peace of mind that someone is always nearby.

The Care Continues

When the moment arrives, we will remain with your family. We’ll notify hospice, or an EMT if hospice is not in place, that your loved one has passed. We’ll also make other notifications if you would like.

Your family may want to gather at the bedside of your loved one for one last time to sit, talk, pray or just remember. This will be your lasting memory, and our caregivers are trained to make this visit a memorable one. We’ll stay with you until you tell us to go.
